นำเรื่อง

แล็บ region นี้มีรายละเอียดมากกว่า region ที่ผ่านมา ดังที่กล่าวในคราวที่แล้วว่าใกล้เคียงกับ upper extremity ทั้งในแง่เนื้อหาและขั้นตอน จึงเป็นธรรมดาที่หลายโต๊ะอาจต้องอยู่ทำแล็บจนเกินเวลา บางครั้งล่วงเลยจนถึงทุ่มหรือสองทุ่มด้วยซ้ำไป

ปัญหาของการอยู่ทำแล็บเกินเวลา นอกจากความเหน็ดเหนื่อยที่เพิ่มมากขึ้นกว่าปกติแล้ว บ่อยครั้งที่อาจารย์ไม่สามารถอยู่แนะนำจนเสร็จสิ้นได้ ด้วยเหตุผลหลายประการ อย่างตัวผมเองก็อยู่เวรที่ห้องฉุกเฉินเป็นประจำ จึงต้องสรุปเนื้อหาให้จบทันตามกำหนดเวลา ส่วนขั้นตอนที่เหลือนั้นคาดหวังว่าน้องๆ ศึกษาจาก lab direction เพิ่มเติมด้วยตนเองได้

เมื่อเวลาเร่งรัดเข้ามาเช่นนี้ จึงเป็นหน้าที่ของน้องๆ ที่ต้องใส่ใจอ่าน lab direction ล่วงหน้ามาก่อน และควรตั้งใจฟัง talk lab ให้ดี เพื่อให้การทำงานราบรื่นและรวดเร็ว ไม่เช่นนั้นย่อมทำให้งานคั่งค้างทับถม กลายเป็นภาระที่หนักเกินความจำเป็น

Antr aspect of Thigh (continued)

Tensor fascia latae m.
- o : ASIS & antr part of iliac crest
- i : ilio-tibial tract
- n. : supr gluteal n.
- action : flex thigh, tense fascia lata

* tensor fascia latae = tensor of fascia lata
- fascia latae เป็น genitive case หรือ possessive case ของ fascia lata
--- ไวยากรณ์พื้นฐานภาษาละติน, น้องๆ ที่สนใจ พูดคุยกันหลังไมค์ได้
--- ตรงกับ ฉัฏฐีวิภัตติ ในการผันคำนาม (แจกวิภัตตินาม) ในภาษาบาลี

Quadriceps femoris m. (four-head m. of thigh)
- ประกอบขึ้นจากกล้ามเนื้อย่อย 4 มัด คือ
--- 1. rectus femoris
--- 2. vastus lateralis
--- 3. vastus intermedius
--- 4. vastus medialis
- ทั้ง 4 มัดรวมกันเป็น common tendon = quadriceps tendon --> base of patella
--- tendon ต่อเนื่องจาก patella --> tibia tuberosity = ligamentum patellae

- n. : femoral n.
- action : extend knee

* จงใช้ความรู้ไวยากรณ์พื้นฐานภาษาละติน อธิบายความหมายของ ligamentum patellae

บางตำรานับรวม articularis genu ไว้ด้วย เพราะเป็น derivative of vastus intermedius
- เพื่อไม่ให้สับสน จำเพียง 4 มัดก่อน ส่วน articularis genu เป็นคะแนนเสริม

1. Rectus femoris
- o : ASIS & ilium supr to acetabulum
- action : ช่วย flex hip ด้วย

2. Vastus lateralis
- o : greater trochanter & latl lip of linea aspera

3. Vastus intermedius
- o : antr & latl surface of shaft of femur

4. Vastus medialis
- o : intertrochanteric line & medl lip of linea aspera

Profunda femoris a.
- เริ่ม femoral triangle <-- femoral a.
--- largest br. of femoral a., chief a. of thigh
- กั้นแยกจาก femoral a. ด้วย adductor longus
- pass infr-ly in medl compartment of thigh, deep to adductor longus
- wrap around postr aspect of femur
- สิ้นสุด : last (4th or 3rd) perforating a.

- Branches
--- 1. medl circumflex iliac a.
--- 2. latl circumflex iliac a.

สังเกต
- br. of profunda femoris a. อาจจะแตกแขนงจาก femoral a. ได้
- desc. br. of latl circum. iliac a. ทอดคู่กับ n. to vastus lateralis

Medial aspect of Thigh

Muscles
Gracilis m. (= slender)
- o : body & infr ramus of pubis
- i : supr part of medl surface of tibia
- n : obtulator n.
- action : adduct thigh, flex knee, help rotate leg medl-ly

สังเกต
- gracilis m. ทอดข้าม hip & knee jt. จึงออก action ได้ทั้ง 2 jt
- มี common tendinous insertion ร่วมกับ sartorius & semitendinosus m.
--- เรียกว่า pes anserinus (goose's foot)
--- ท่องกันไปเรื่อยเปื่อยว่า "ซาร์-กรา-เซ็ม (Sar-Gra-Sem)"
----- ขอแนะนำให้จำว่า "Sar-Gra-Ten" จะดีกว่า เพื่อไม่ให้สับสนกับ semimembranosus

- pes anserinus ประกอบขึ้นจากกล้ามเนื้อ 3 compartments
--- supplied ด้วย n. ต่างกัน
--- 1. sartorius : antr = femoral n.
--- 2. gracilis : medl = obtulator n.
--- 3. semitendinosus : postr = tibial n. (of sciatic n.)

Pectineus m.
- o : supr ramus of pubis
- i : pectineal line of femur
- n. : femoral n. & obtulator n.
- action : adduct & flex thigh, medl rotation of thigh

สังเกต
- pectineus แบ่งเป็น 2 layers; supplied ด้วย n. ที่ต่างกัน; action ต่างกัน
--- 1. supfl : femoral n.
--- 2. deep : obtulator n.

Adductor group
- n. supply : obtulator n.
- main action : adduct thigh

1. Adductor longus
- o : body of pubis infr to pubic crest
- i : middle 1/3 of linea aspera of femur

2. Adductor brevis
- o : body & infr ramus of pubis
- i : pectineal line & prox. part of linea aspera

3. Adductor magnus
แบ่ง 2 parts; ต่างกันที่ตำแหน่ง, n. supply & main action
- 3.1 adductor part
--- o : infr ramus of pubis & ramus of ischium
--- i : gluteal tuberosity, linea aspera, medl supracondylar line of femur
--- n. : obtulator n.
--- action : adduct & flex thigh

- 3.2 hamstring part
--- o: ischial tuberosity
--- i : adductor tubercle of femur
--- n. : tibial n. (of sciatic n.)
--- action : adduct & extend thigh

Adductor opening/hiatus
- ช่องระหว่างรอยต่อ adductor part & hamstring part of adductor magnus m.

Obtulator externus
- o : margin of obtulator for. & obtulator membrane
- i : trochateric fossa of femur
- n. : obtulator n.
- action : latl rotate thigh, พยุง head of femur ใน acetabulum

Nerve
Obtulator n.
- เริ่ม : antr division of antr rami of L2-4
- desc. along medl border of psoas m.
- ผ่าน obtulator for. เข้าสู่ thigh
- สิ้นสุด : แตกแขนงเป็น antr & postr br. คร่อม adductor brevis m.

สังเกต
- obtulator n. แตกแขนงคร่อม adducter brevis m. เพราะฉะนั้น
--- antr br. ทอดระหว่าง adductor longus & brevis m.
--- postr br. ทอดระหว่าง adductor brevis & magnus m.

"...ในกรณีเช่นนี้ หากจะกล่าวว่าแพทย์จบใหม่ร้อนเงินตามกระแสทุนนิยม คงจะเป็นการกล่าวที่เกินเลยไป และเป็นการโยนบาปให้กับแพทย์ไปโดยรู้เท่าไม่ถึงการณ์ เพราะแท้ที่จริงแล้วปัญหาไม่ได้อยู่ที่แพทย์ แต่อยู่ที่การจัดสรรตำแหน่งทุนของกระทรวง สธ. ที่ยังไม่สอดรับกับความต้องการของประชาชน..."
