Stomach; a breif concept
Review Aj Yadaridee's lecture, as obligated !!

J-shaped, 4 parts
- 1. cardia
- 2. fundus
- 3. body
- 4. pylorus : antrum, canal, sphincter

Borders
- 1. lesser curvature ต่อเนื่องกับ angular notch
- 2. greater curvature ต่อเนื่องกับ cardiac notch

Orifices
- 1. cardiac : gastro-esophageal junction มี LES
- 2. pyloric : gastr-duodenal junction มี pyloric sphincter

Stomach bed (relate to postr surface)
- trans. mesocolon & trans. colon
- gastric surface of spleen & splenic vv.
- antr surface of body & tail of pancreas
- Lt. kigney & Lt. suprarenal gl.
- diaphragm & peritoneum of pstr abdl wall

Arterial supply
- 1. Lt. gastric a. <-- coeliac trunk
- 2. Rt. gastric a. <-- hepatic a. proper
- 3. Lt. gastro-epiploic a. <-- splenic a.
- 4. Rt. gastro-epiploic a. <-- gastro-duodenal a.
- 5. short gastric a. <-- splenic a.

สังเกต
- a. 1 & 2 lie in lesser omentum อยู่ชิด lesser curvature
- a. 3 & 4 lie in greater omentum ห่างจาก greater curvature เล็กน้อย
- a. 5. supply fundus มีหลายเส้น

Nerves
- parasymp. from vagus n. : กระเพาะหดตัว, sphincter คลายตัว

- symp. form coeliac gg. <-- thoracic splanchnic n.

- ย้อนขึ้นไปที่ GE junctn. ให้ esophageal br. เลี้ยง abdl esophagus

Splenic a.
- แขนงใหญ่ที่สุด; คดเคี้ยว ทอดตัวตาม supr border of pancreas
- เข้าใน spleno-renal lig. แขนงปลายเข้า hilus
- branches
--- 1. splenic brs. เข้า hilus
--- 2. pancreatic brs. แตกแขนงมาตามทาง
--- 3. short gastric a. เลี้ยง fundus of stomach

ล่างต่อ coeliac ประมาณ 1 cm.; lower part of L1 vert.

สังเกต
- a. 2 & 3 แตกแขนงทางด้านซ้าย
- a. 4 - 6 แตกแขนงทางด้านขวา


Infr pancreatico-duodenal a.
- แตกแขนงเป็น antr & postr brs ทอดตัวไปตาม head of pancreas; corresponding sides
- anatomose with antr & postr supr P-D a., respectively
- supply head & uncinate process of pancreas


Jejunal & ileal a.
- แตกแขนงทางซ้าย; numerous branches
- form anastomosing arcades to supply the small intestine
- The number of arterial arcades increases distally along the gut
- ทบทวนความแตกต่างของ arterial supply ที่ jejunum & ileum


Middle colic a.
- เข้าใน trans. mesocolon; supply trans. colon
--- Rt. br. anatomoses with Rt. colic a.
--- Lt. br. anatomoses with Lt. colic a. (<-- IMA)


Rt. colic a.
- เข้าใน retroperitoneum; supply the ascending colon
--- asc. br. anatomoses with middle colic a.
--- desc. br. anatomoses with ileo-colic a.


Ileo-colic a.
- ลงไปทาง Rt. iliac fossa
--- supr (asc) br. anatomoses with middle colic a.; supply asc. colon
--- infr br. ทอดตัวมาทาง ileo-caecal junctn; 4 แขนง

----- appendicular br. ผ่านมาตาม free edge of meso-appendix

Sigmoid a.; 2 - 4 branches
- descend to the left, in the sigmoid mesocolon
- supply the lowest part of the desc. colon and the sigmoid colon

Supr rectal a.
- ลงไปใน pelvic cavity, in the sigmoid mesocolon

ท้ายเรื่อง

วันนี้ผมอยู่เวร ER จึงต้องเลิกแล็บตรงเวลา แต่ก็พยายามทบทวนเนื้อหาให้เท่าที่เวลาจะอำนวย โชคดีอยู่บ้างที่เนื้อหาในวันนี้ไม่มากจนเกินไป และน้องๆ ก็เร่งมือกันจนทำแล็บเสร็จก่อนหมดเวลาตั้งนาน

หลังจากนี้ไปอีกหลายสัปดาห์ วันพุธที่เราเคยว่างกันคงไม่มีอีกแล้ว เวลาพักผ่อนของน้องๆ ก็คงลดน้อยลงไปตามตารางเรียนที่ชดเชยอัดแน่นกันเข้ามา แม้แต่คาบทัศนศึกษาในวิชา SIID ของพวกเราก็ถูกลดทอนลงไปอย่างน่าเสียดาย ชวนให้รู้สึกราวกับว่าการเรียนหนักขึ้นอีกหลายเท่าตัว - ไม่เฉพาะแต่น้องเท่านั้นนะครับที่ต้องเหนื่อย อาจารย์ทุกท่านก็ทำงานหนักขึ้นเช่นกัน

ฉะนั้น ขอให้น้องๆ ตั้งสติให้ดีครับ ค่อยๆ ปรับตัวกับความเปลี่ยนแปลง จัดสรรเวลาให้คุ้มค่าทั้งการเรียนและการเล่น อย่าหนักไปในทางใดทางหนึ่งมากจนเกินไป หากมีปัญหาใดที่ผมพอจะช่วยเหลือได้บ้างก็ยินดีเสมอครับ
